You may now have your doctor submit your application to us via email. With new technology available many doctors offices have this option either with their printers or through their computer systems. Please don't be afraid to ask them if this option is available. You can do so one of the following ways:
1. Have your printed application with you at your initial appointment. Have your information filled out and have the doctor fill out his required areas. Please ask your doctor or their staff to digitally scan your signed application and email it to us at: [email protected]
2. Have your doctor or their staff visit this website and print out the application for you. Both of you please fill out your required information and have the doctor or their staff digitally scan your application and have them email it directly to our email shown above.
Note: Our eventual goal is to have a secure, online application available for doctors to be able to access via this website. We are currently exploring the best options to do this and we will keep you updated with any new news.
